CLIENT RECEPTION: Health & Safety Registration for Guests
The Health & Safety Registration for Guests sign in sheet is required for all guests who visit Veritext offices. Please collect each person’s contact information so that we may alert any guests that may have been exposed to COVID.
- Please download, print and place in a binder several copies of the “Health & Safety Registration for Guests” sheet.
- Maintain sanitization of pens used to fill out the registration.
- Keep past registration records within the binder for one month, then shred.
